Registration Rules:

A) The player named on the registration form is registered and insured with KDMHA ONLY AFTER:            

1.   The form has been properly and fully completed.            
2.   The form has been signed by the player (or his/her parent/guardian).            
3.   The form has been signed by the Registrar.
4.   The registration form has been entered into the ODMHA (Hockey Canada) registration database for Panther Players.  

All completed registration forms and monies must be received and paid in full by August 1, 2008. After August 1, 2008, the following applies:

1. A late fee of $75.00 per player will be charged to the full season payment.   This fee will be waived for new players registering in the association.
2. An administration fee of $25.00 per player will charged for all refund requests.   This fee will be waived for players named to a higher level team within District 1 and declared prior to September 15, 2008. (e.g. Rideau St. Lawrence Kings)
3. Player names will be placed on a waiting list if their designated level is deemed ‘full’.

The following CSA approved equipment is to be properly worn:

Players:

• Helmet with face mask and chin strap                                                
• Hockey Skates                                    
• Mouth Guard (blue in colour not clear or red)
• Athletic support and cup                        
• Hockey Pants                        
• Shoulder Pads
• Elbow Pads
• Shin Pads
• Hockey Gloves
• Neck/Throat Protector (BNQ) Approved
• Sweater & Socks                       

Goaltenders:

Chest Protector and Additional Arm Pads Both Neck/Throat Protector (BNQ) Approved Throat Guard attached to the face mask.
