Coach Survey

As the 2011–2012 regular season draws to a close, the KDMHA is asking members to complete the annual Coach Evaluation Survey. This is a valuable tool for the KDMHA and Coach Coordinator to help assess the success of our coaches and see where additional training or support may be required.

The results of this survey are completely anonymous; however there is a spot for your to put your name if you would like to discuss your concerns or comments in greater detail.

Please remember it is just as IMPORTANT to provide POSITIVE feedback as it is to provide concerns. These survey results are shared with the coaches at the end of the season (remember all responses are anonymous and only overall totals are provided), and are used by the Coach Selection Committee as back ground information.
