Application Screening Process

OPP screening form that needs to be completed for all team officials (coaches, ass’t coaches, trainers, managers, etc.).  The screening needs to be completed annually and even if you completed one earlier this year for some other purpose (coaching soccer, baseball, etc.), a separate screening needs to be completed specifically for KDMHA.

Process to follow:

Complete ‘Applicant Information’ and ‘Previous address for the last 5 years’ (if applicable). ‘Name’ and ‘Title’ at the bottom of the 1st page can be left blank. For ‘Name of Organization’ put Kemptville and District Minor Hockey Association. On the 2nd page, only ‘Signature of Applicant’ and ‘Date’ are required – all other areas/sections can be left blank.

Take the completed form to the station during business hours (8:00am-4:30pm) with two pieces of id (e.g., license, birth certificate, etc.). Sometimes after hours are available for drop-off/pick-up so call to check (258-3441).
